Informed Consent Statement for Clients

Services Offered:
Sarah Gewanter is currently providing a variety of services including, but not limited to: hypnosis, meditation, NLP (Neuro Linguistic Programming), EFT (Emotional Freedom Technique “Tapping”), EMI (Eye Movement Integration), energy work, mentoring, and life coaching. Her approach is holistic and positive to help guide you to reach your goals, succeed, thrive and move forward along your life path. Ms. Gewanter does not practice or treat clients as a clinical social worker. She does not evaluate or treat medical or mental health illnesses. She helps people deal with everyday life issues in as fast and effective ways possible.

Fees:
Sarah Gewanter does not offer 3rd party insurance reimbursement nor does she bill insurance companies for her services. All fees for service are private pay and are to be paid at the time of the service unless other arrangements are made. Sliding scale fees are available; if needed, please discuss.

Responsibility:
You are responsible for your wellness and progress. If there is anything you do not agree with or are uncomfortable with, please discuss with Sarah Gewanter - she is very open to feedback and committed to making your experience as positive as possible.

I have read and understand the policies and agree to the conditions. I agree to the statements herein and terms of payment, to include payment of all fees listed. If guardian for a minor client, I certify that I have the legal right to consent to sessions on the child's behalf.

I consent to participate in the sessions offered to me by Sarah Gewanter. I understand that I may withdraw my consent and stop sessions at any time.
